Visiting Sister for the holiday season. Happened to be watching T.V. and
they were showing "Where no man has gone before".

One of the Crew, Lt Gary Mitchell, gains super (God-like) powers, as
does Dr Elizabeth Dehner (eventually). In the near-final scene,
Elizabeth semi-disables Gary, so Kirk can have some chance in the fight,
but Gary regains his super-powers and creates a "grave" for Kirk,
complete with headstone that reads "James R Kirk"

Given this was an early episode (or second Pilot), was this prior to a
decision being made to go with "James T Kirk" or was it a simple typing
error (the "R" is next to the "T" on a keyboard) and money was so tight
that nobody bothered fixing the typo??

This was the first episode. It did indeed come prior to the decision
to make Kirk's middle name Tiberius.

Gary Mitchell may have had super powers, but he didn't necessarily
know Kirk's middle name. ;)

Short answer: Nobody knows for sure.

There really isn't any doubt that Kirk's name was always James Tiberius
Kirk, since the early show bibles mention it, and, IIRC, "Tiberius" was
the middle name in place while "James" and "Kirk" were still in flux.

So I'd go with 'mistake'

And here I thought this was something that came into being from a
letter of Gene Roddenberry to a fan after the original series had

Nope.The Kirk parents called their twin sons "James T Kirk" and "James R
Kirk", so that really the grave of Enterprise Kirk's twin brother
(unfortunately he was the one who could act and sing). ;-)

It was the second pilot, and many things were different:
no Dr. McCoy, different weapons, different junior officers
etc. No need to have an extensive "bible" until the series
was approved. That's why the things/people in it aren't
identical to the first pilot either.
